Gluten-free products have exploded from a medical necessity for a small population into a multi-billion-dollar industry marketed to everyone. But is going gluten-free actually healthier for people without celiac disease or gluten sensitivity? The science has some surprising answers.

What Is Gluten?

Gluten is a family of proteins found naturally in wheat, barley, and rye. It gives dough its elasticity and bread its chewy texture. For most people, gluten is completely harmless and easily digested. It becomes a problem only in specific medical conditions.

Who Actually Needs to Avoid Gluten?

Celiac disease affects approximately 1% of the global population. It is an autoimmune condition in which gluten triggers an immune response that damages the small intestine's lining. People with celiac disease must strictly avoid gluten for life. Non-celiac gluten sensitivity (NCGS) affects an additional 0.5–13% of people and causes gastrointestinal symptoms without the intestinal damage. Wheat allergy is a separate immune response to wheat proteins, including but not limited to gluten.

The Gluten-Free Product Trap

The irony of the gluten-free food market is that many products are nutritionally inferior to their gluten-containing counterparts. To replicate the texture and taste of wheat, manufacturers often add extra sugar, fat, starch, and additives. A 2017 review in Epidemiology and Psychiatric Sciences found that gluten-free products in the UK contained significantly more calories, fat, and sodium, and less protein and fiber than their standard counterparts.

Whole Foods vs. Processed Substitutes

Naturally gluten-free whole foods — rice, corn, quinoa, potatoes, legumes, fruits, vegetables, and most animal proteins — are excellent choices for anyone, celiac or not. The problem is when people replace whole wheat bread with processed gluten-free bread loaded with refined starches and additives. If you feel better eating less gluten, it may be because you're eating fewer processed foods overall, not because you've eliminated gluten specifically.
